Understanding Your T4A and the CDCP form

You may have received an eligibility form from the CDCP based on the dental coverage access information reported in Box 15 on your T4A. Here’s what it means and how the code on your T4A applies to you.

What the T4A code means

• The code on your T4A is correct and shows your access to dental coverage through the pension plan.
• It does not reflect whether you were actually enrolled or eligible.
• If you maintained dental coverage with another comparable provider, you may still be eligible to enroll in the pension plan’s dental plan and have access to coverage.
• If you didn’t maintain continuous coverage and are no longer eligible, you are still deemed to have access because the pension plan does not have information about your other coverage.

Why this is on your T4A

The Canada Revenue Agency now requires pension plans to report whether members, their spouses, and dependents had access to dental coverage each tax year. This information is shown in Box 15 on your T4A and reflects each plan member’s status in their pension records.
