Canada Dental Care Plan Eligibility form

If you are a retired member who has received the Canada Dental Care Plan Eligibility form:

  • Complete the applicable section
  • Scan pages 1 and 2
  • Upload PDF using Message Centre in My Account
    Note: Processing may take up to 14 days

The code in Box 15 on your previous tax slip (T4A) is correct. This means members have access to dental coverage even if they are no longer eligible to enrol.
